This E blog also reflects my patient centered online learning portfolio CHIEF COMPLAINTS:-   A 52 old female has come to the casuality with chief complaints of  Fever and cough since 1 week Shortness of breath since 3 days  HISTORY OF PRESENT ILLNESS Patient was apparently asymptomatic 15 years back and was diagnosed with diabetes  under OHA Tab Glycomen gp1 for 2 years 6 months back patient had an episode of fall  Patient noticed pedal edema Shortness of breath Burning micturition and  Facial puffiness for 2 days She was taken to a private hospital and was diagnosed to have increased creatinine levels (7.5) Patient was

I’ve been given this case to solve in an attempt to understand the topic of “patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations, and come up with diagnosis and treatment plan.
